The Diabetes Educator 31:849-857, wish to indicate that Peggy B.

Smith's affiliation is with the Division of Endocrinology, Diabetes,
Nutrition, and Metabolism and Department of Health Care Policy and Research,
Mayo Clinic, Rochester. The authors also wish to acknowledge the efforts of
